Dr. Smith's evaluation includes a 30-minute parent conference before testing, testing, report, and a 1-hour parent conference after completion

Dr. Smith will complete two interest inventories, evaluate true job placement options, and offer data on realistic wages for your major.

Alongside one's mortgage, a college education represents a large and significant cost. Will there be a return on investment?
Dr. Smith will compare colleges, probability of employment, and post-graduation wages and provide students and parents with a true understanding of their investment.

Dr. Smith offers regular meetings of a support group for parents with varying disabilities. The groups consist of 5-8 single or sets of parents. Meetings last 1 hour and 20 minutes. Cost is $10 per meeting. If a set of parents comes, the cost is $5 per parent.

Individuals with disabilities often have difficulty returning to work with an ongoing or sudden-onset disability.
Dr. Smith offers a complete vocational interview, vocational testing,
and a report to assist these individuals in their return to work.
Minimum charge of $750 per report
Dr. Smith can accommodate the educational and vocational evaluation of individuals with blindness and/or visual impairment including modified testing to incorporate special needs.
Minimum charge of $1,500 per report
In the scope of employment or outside of the workplace, individuals can experience physical, cognitive, psychological, or other types of injuries, wherein, a return-to-work plan may not
be straightforward.
Dr. Smith will meet with the employee and employer to complete a job analysis and evaluate return-to-work processes.
$150 hourly rate; minimum $600; total cost will be based upon need.
Employers often require educational outreach for employees regarding a particular topic pertaining to disability in the workforce.
From understanding autism and Asperger's related symptoms to comprehending age-related changes in employees, and beyond, Dr. Smith can accommodate topics based on need.
Typical charge - $250 to prep and present a 1-hour presentation.
After an injury, a worker may have difficulty navigating the potential medical maze toward recovery and rehabilitation.
Dr. Smith will coordinate and schedule appointments, offer patient education, and report back to an employer about the worker's medical progression.

Individuals with traumatic brain injury offer a unique challenge when he or she returns to the workplace. Both employer and employee most often misjudge the emotional, cognitive, and social adjustment needed for a person with a traumatic brain injury to return to the workforce.
Dr. Smith will complete a job analysis for someone's work duties, complete an interview with the employer and employee, review medical records, and offer return-to-work recommendations.
$150 per billable hour;
billing 1/4 segments of time.
Dr. Smith offers vocational interviews, intelligence and academic aptitude testing, and custom reports for personal injury vocational evaluation. $250 hour, minimum $1,500; Deposition and Court Rate, $550 hour, minimum 3 hours. I would estimate most plans range from $1,500 to $3,500. Does not include deposition or court costs.
An assessment of the long-term medical, vocational, educational, and social needs is warranted for litigation or development of trusts.
Dr. Smith develops a report based on past, present, and future needs. Cost is based upon the extent of disability and range of medical needs. Does not include deposition or court costs.
Life Care Plans include a summary of past medical events, a listing of projected medical needs, and a summary of costs will be provided.
Dr. Smith may or may not complete an interview with an injured worker or plaintiff, based upon need and/or availability.
$250 per hour; estimated total costs will be based on need. Does not include deposition or court costs.
At times a complete Life Care Plan is not needed. In these cases, Dr. Smith can provide an attorney with an estimate of projected medical costs, based on physician direction.

Occasionally, a voluminous amount of medical, psychological, or educational records need to be categorized, and summarized.
Dr. Smith can organize the records by physician, date, disability, or other categories and offer a chronological listing. Cost is $3 per page;

Within a divorce situation, information is needed about potential income, lost employment opportunities, or other factors related to employment for previous spouses.
Dr. Smith will evaluate one's educational profile, examine current labor market trends, and offer data regarding potential and lost wages. A report will be generated.
$125 hour; minimum 6 hours. Does not include deposition or court costs.
